    • Housing Case Manager

    • MISSION:
      To provide subsidized housing opportunities and linkage to support services for residents in the Shoreline, with a goal of ending homelessness.

      SCOPE:
      To provide support to individuals applying for and or receiving housing subsidies including but not limited to HOPWA, RAP, Next Steps, and SHP.

      QUALIFICATIONS:
      Bachelor’s Degree – experience working with homeless population preferred.

      HOURS
      37.5 hrs/wk Monday through Friday 8:30AM to 4:30PM

      RESPONSIBILITIES
      • Assist individuals with the application and eligibility process for designated supportive housing programs funded through HUD, DMHAS, HOPWA, HAF and other funding sources. Work with applicants to complete application for housing assistance, including obtaining documents and identifications.
      • Assist in determining basic eligibility of supportive housing programs applicants.
      • Assist homeless individuals with all aspects of housing search and placement in accordance with BHcare supportive housing programs including working with clients to devise an appropriate housing search strategy if applicable.
      • Negotiating with landlords and realtors as needed. Advocate for individuals as indicated
      • Maintain regular contact with supportive housing program participants through home and office visits, case coordination with other providers, and phone calls.
      • Support program participants in making the transition from homelessness to permanent housing by assisting them in maintaining the terms of their lease agreement, program guidelines and individual service plans.

    • Per Diem Residential Counselor

    • RESPONSIBILITIES:

      -Assess clients’ functionality and mental health status, needs and preferences, and ongoing appropriateness of services.

      -Assist clients in developing and implementing individual recovery plans.

      -Ensure appropriate referrals and linkages are made to other parts of the mental health system and other community resources, including clinical, vocational, psychosocial, educational and natural supports.

      -Demonstrate the ability to understand the MRO (Medicaid Rehab Option) process, the delivery service required for TCM (Targeted Case Management), and the IDDT (Integrated Dual Diagnosis Treatment) principles utilizing Motivational Interviewing.

      -Demonstrate an ability to develop a basic level of competency for providing services to clients with co-occurring disorders.

      -Assist all Program clients in the areas of health and safety and in the acquisition of independent living skills, including personal hygiene, meal planning, cooking, cleaning and money management.

      -Provide rehab interventions as directed by resident recovery plan, including facilitating rehab groups.

      -Provide transportation.

      HOURS:

      As needed. The Per Diem Residential Counselor is not guaranteed any number of hours, however, he or she is required to be available to work a minimum of 16 hours per month if called.

      SHIFTS
      1st – 8:00am – 4:00pm
      2nd – 4:00 – Midnight – waking hours
      3rd – Midnight – 8:am – waking hours

      LOCATION: Branford, CT

      REQUIREMENTS: Two years of college education (human services or behavioral science preferred) or high school diploma and equivalent direct care work experience. Must maintain an acceptable driving record and be insurable at a reasonable rate under the Agency’s auto insurance plan.

    • Recovery Support Specialist

    • RESPONSIBILITIES:

      Participates in the ongoing development of a Recovery Supports program for people in recovery from substance use and other disorders.

      Assists peers with advocacy issues either individually or in a group setting.

      Coordinates workshops and self-help activities (skills training and/or social).

      Sets up speakers for topics or presents topics.

      May include coordination of materials for workshops.

      Encourages clients to engage in services that will assist them in their recovery.

      Encourages and assists clients in becoming active participants in their community.

      Links clients with community mutual support organizations such as AA/NA and other appropriate support groups, specialized inpatient/outpatient and/or treatment centers.

      Assists clients in securing emotional, vocational, social supports as determined by the clients.

      Assists clients in accessing services as requested by the client, including housing, employment and benefits.

      Works with other staff members to provide crisis management screening and intervention, utilizing clinical and emergency back-up as indicated.

      Assists clients in program with necessary transportation needs.

      Attends advocacy and recovery community organization meetings as appropriate.

      Has working knowledge of MRO process, the TCM delivery system and the IDDT principles.

      HOURS:25 hours per week, Monday – Friday

      LOCATION: Branford, CT

      REQUIREMENTS:

      Previous volunteer or work experience with adults with substance abuse issues strongly preferred.

      High school diploma required; Bachelor’s Degree preferred.

      Experience with individual and group treatment settings preferred.

      Must possess or have the ability to obtain and maintain a Public Passenger endorsement.

      Personal experience with recovery strongly preferred.

    • Social Rehabilitation Supervisor

    • RESPONSIBILITIES
      • Responsible for the daily operation of the program including but not limited to: staff scheduling, overseeing petty cash, facilitating staff meetings and the monthly calendar.

      • Coordinate program intakes and assign clients to staff.

      • Provide one-on-one supervision in accordance with agency policy.

      • Upon intake, assess client’s abilities and mental health status, needs and preferences, and ongoing appropriateness of services.

      • Provide crisis assessment and intervention, utilizing clinical and emergency back-up as indicated. Communicate client needs with hospital emergency rooms and emergency personnel.

      • Assist clients in developing and implementing individual recovery plans and reviews. Coordinate recovery planning to include members of clients’ treatment team and clients’ natural supports.

      • Demonstrate the ability to understand IDDT (Integrated Dual Diagnosis Treatment) principles utilizing Motivational Interviewing.

      • Assist staff in obtaining a basic level of competency for providing services to clients with co-occurring disorders.

      • Oversee the production of the monthly calendar of activities, incorporate interdepartmental activities.

      HOURS
      8:30 – 4:30 some nights and weekends

      REQUIREMENTS

      Bachelor’s Degree in Rehabilitation or related field.

      Must Have the ability to obtain and maintain a Public Service License
